Funny, but the real reason is "Requirements Creep" where the design requirements keep changing and ripple through the whole design and you have to either rework or redesign while assemblies very late in the game. This in turn causes the vendor huge headaches to make all these changes very late in the game. This is why the costs spiral out of control and schedules get impacted. I would blame the goverment oversight more than the contractor!
